Attorney Medical Records Request

The following form is intended for use by law firms only to request their clients’ medical records.

Premier Urgent Care processes medical records requests only on Thursdays. All submissions will be directed to our custodian of records. Records are delivered electronically only (via email or fax) and will include a certification signed by the custodian.

Please note : Payment is required at the time of your request.

Records will not be processed until payment has been made in full.

Checks will not be accepted.

Typical processing time for medical records is up to 15 business days. However, if you select expedited service, most records will be prioritized and sent electronically within 5 business days.

Regular Service - $45 Expedited Service - $75
The following form is intended for use by law firms only, to request their clients' medical records. Premier Urgent Care only processes medical records on Thursdays, and all requests entered into the following form will be directed to our custodian of records. Medical records from Premier Urgent Care will only be processed electronically, via email or fax, and will include a certification signed by the custodian. Payment is required for records to be processed. Checks will not be accepted.

After submission of this form, you will receive an email from our custodian of records, stating the amount of pages you will be receiving. You will then be given a link to make payment for records. After payment is received, records will be processed.

You may choose to expedite your records, for an additional $75 fee.

Expedited records will be processed in 5 days or less. Non-expedited records may take up-to, but no more than, 15 days.

Records are only processed on Thursdays.
