Very Urgent – Attorneys Medical Records Request

Description

The following Service is intended for use by law firms only, to request their clients’ medical records. Premier Urgent Care only processes medical records on Thursdays, and all requests entered into the following form will be directed to our custodian of records. Medical records from Premier Urgent Care will only be processed electronically, via email or fax, and will include a certification signed by the custodian. Payment is required for records to be processed. Checks will not be accepted.
